Outpatient Rehabilitation
CHRISTUS Santa Rosa outpatient rehabilitation centers offer comprehensive outpatient rehab services for children and adults. Our goal is to help patients achieve their highest level of function following an injury, illness or disability.
We offer an integrated, holistic approach to therapy, combined with exceptional medical expertise. Each individualized treatment plan addresses the physical, social and emotional needs of our patients and their families. Our services range from individual service to multi-service, depending on the patient’s specific needs.
Get a Referral
CHRISTUS Santa Rosa outpatient rehabilitation centers accept referrals from physicians, social workers, rehabilitation nurses, case managers and counselors, as well as from patients and their families. Referrals require a written physician prescription.
