Inpatient Rehabilitation
Discharge and Outpatient Treatment
Discharge planning begins at the time of admission. The rehabilitation team will help you make a smooth transition from the hospital back into your home and community.
Home evaluations. If needed, your therapist will visit your home to identify any problems you may encounter there. Before you go home, your therapist will teach you any skills required and recommend any home modifications to help in the transition back to home. We will also provide training for family members who may need to assist you once you return home.
Assistive devices and equipment. Your therapist may recommend equipment to help you in various activities of daily living. We will provide equipment to practice with during the rehabilitation stay. If you need equipment once you return home, we will help you order it before you leave the hospital.
Outpatient therapy. We plan for and provide continued specialized outpatient services, if ordered by your doctor, after you have been discharged. CHRISTUS Santa Rosa rehabilitation services include a comprehensive outpatient therapy program.
